Skin Wrinkles
Wrinkles are the inevitable effect of living in Australia's sundrenched environment.
Wrinkles are of two types - static ( that is, the wrinkle is present when the face is expressionless) and dynamic (those creases resulting from squinting, raising the eyebrows, frowning and even chewing). Government regulations control the advertising and promotion of products described as "prescription only". They include all those injectable materials that are commonly used to treat wrinkles in a temporary or semi-permanent way.
Wrinkle Removal
Dynamic wrinkles can be suppressed or even eliminated by the periodic injection of muscle relaxants. There are several preparations of this chemical group available commercially. At the Sydney Institute of Skin Rejuvenation our preference is for that product most widely known and used. The injection is administered directly into the offending muscle area with a very fine needle to achieve accurate placement. The treatment takes but a few minutes and should be painless. Within a few days, the muscle action diminishes but the full effect is seen about 7 days after the injection.
Expertly done, the beneficial effect should last from 4-6 months. Results depend on several factors, such as the potency of the material when injected, the skill of the operator and the metabolism of the patient.
To maximize the benefit of the treatment, Dr Olbourne, our principal, prefers to perform the injections himself. The Sydney Institute of Skin Rejuvenation invites our patients seeking wrinkle injections to attend as a group, so that the reconstituted product is as powerful as it can be. Patients are advised of our injection dates and choose to attend for subsequent treatments when they notice the effectiveness of their last injection is beginning to wane. Regular top-up injections at 3-4 month intervals have bee shown to deliver the most effective ongoing results.
Wrinkle filler
Static wrinkles are those that remain even when there is no animation of the face. They result from a gradual loss of the elasticity of the skin with age and are exacerbated by the sunny environment in which we live. They most noticeably affect the lip, the marionette area from outer edge of lip to chin, and the nasolabial furrows which run from the edge of nose to the outer lip.
Many of these lines and wrinkles can be improved by tissue fillers with which we at the Sydney Institute of Skin Rejuvenation have an extensive experience.
Temporary tissue fillers are hyaluronic acid based. They are introduced into the skin at different levels with the finest of needles. They fill the depression that causes the wrinkle and can then absorb naturally existing moisture into the area by virtue of their chemical structure. This action can cause a further controlled swelling of the treated area thereby improving the result of the injection. Hyaluronic acid is a normal constituent of dermis, one of the deepest tissues in the skin. Like all other proteins in our body, it is in a constant state of turnover. However, being a material not recognized as self, it is not rebuilt by the body and therefore is not a permanent solution for the wrinkle. It is however, simple, available and effective, with a duration of effectiveness of more than six months. These injectable fillers are an excellent solution for creases and skin depressions and to enlarge the lips and cheeks.
Permanent fillers used at the Sydney Institute for Skin Rejuvenation include your own fat or other forms of synthetic preparation. Endogenous fat is harvested from the abdomen, buttock or inner knee, and after cleansing and rinsing can be re-injected into wrinkles, dimples or the lips. It has the advantage of being your own material, and therefore not subject to allergic considerations or rejection. However, it requires some form of anaesthesia (local or general) to ensure patient comfort, and involves more down time than an injectable filler. The improvement from a fat injection may last for months, or even years, but the duration of effect can be a little unpredictable because of the unique nature of each individual patient and their fat, which may persist or dissolve away in a manner which can not be predicted or controlled.
The non-resorbable, long lasting, injectable filler which we favour usually yields permanent results. The component gel of the injection, which has been used safely and effectively in well over 30,000 patients in more than 10 years, is not composed of natural body proteins and is therefore not reabsorbed. It is intensely hydrophilic (meaning it attracts water to it like a sponge) and therefore gives an immediate result which improves over a short period of time. The treatment is performed on an outpatient basis under local anaesthesia and sterile conditions by our principal surgeon, Dr Norman Olbourne.
Lip enhancement
can be achieved either by the injection of the dermal fillers alone or, if indicated, in combination with surgery. The temporary injectable tissue fillers can be used in an incremental and repeated way to gradually increase the bulk of the lips and produce an aesthetically pleasing increase in volume and definition of the lips. Once a satisfactory result has been achieved, a permanent material can be injected to achieve a controlled and previously agreed outcome.
For more permanent results non-resorbable materials including your own fat can be utilised to achieve the same result, but require a short day stay in surgery and a slightly longer recovery period.
Lip Shortening
In some patients, thin lips are associated with the generalized elastosis of ageing. For these patients a surgical procedure to shorten and evert the upper lip can produce remarkable and satisfying results even at the cost of a day-stay surgical procedure and a slightly longer recovery period.
